Service Agreement
This Service Agreement (“Agreement”) is being made between —— (that’s “You” or “Your”) and Juan P. Osorio (that’s “We” or “Our”) on 12/10/21. ———–(“You”) and Juan P. Osorio (“Our”) may also be referred to as “Party” or together as the “Parties”. This Agreement will become effective on 12/10/21 (“Effective Date”).
1. Services
We agree to perform the services (“Services”) listed in this Section 1. The Parties acknowledge that their obligations pursuant to this Agreement serve as good and valuable consideration for this Agreement.
a. Services
i. 16-week Ultimate Body Transformation package commencing on the 12/10/21
ii. A minimum of 16 weeks’ coaching with Juan P. Osorio of JP Osorio FIT.
iii.Unrestricted access to Juan P. Osorio and his team via the channel WhatsApp and via email.
iv.One weekly coaching call with Juan Osorio and/or one of his team directly involved with running your programme
vi.A minimum of 3 nutrition plans throughout your 16 weeks – tailored to your goals
vii.A minimum of 3 training plans throughout your 16 weeks – tailored to your goals
viii.We agree to work with you until you achieve the agreed goals outlined in your initial consultation form, even if this is beyond the agreed term. This work is undertaken free of charge

3. Payment
Payment/Payments must be made using the payment link or email transfer sent in the welcome email.
4.Term
The term of this Agreement is 16 weeks from the Effective Date, unless terminated earlier for other reasons available in this Agreement.
5. Termination
This Agreement will automatically terminate when both Parties have performed all of their obligations under the Agreement and all payments have been received.
6. Relationship of the Parties
No Exclusivity. The Parties understand that this Agreement is not an exclusive arrangement. The Parties agree that they are free to enter into other similar agreements with other parties.
7. Waiver
Neither Party can waive any provision of this Agreement, or any rights or obligations under this Agreement, unless agreed to in writing by the Parties. If any provision, right, or obligation is waived, it’s only waived to the extent agreed to in writing.
8. Amendments
This Agreement may be modified as needed. To make a modification, the Parties have to agree to the modification in writing (an “Amendment”). The terms of this Agreement will apply to any Amendment the Parties make.
9. Entire Agreement
This Agreement puts the Parties entire understanding of the Services to be performed and anything else the Parties have agreed to. This Agreement supersedes any other written or verbal communications between the Parties.
10. Severability
If any section of this Agreement is found to be invalid, illegal, or unenforceable, the rest of the Agreement will still stand.
11. Notices
All notices under this Agreement must be sent by email with read receipt, or by certified or registered snail mail with return receipt requested.

By signing below, the Parties agree to the terms of this Agreement. This document is considered as signed when You print your name, date the form and return to jposoriofit@gmail.com or when replying to the email stating ‘I accept the terms of the agreement’
